Overview Of Falcon Mews
Falcon Mews is a supported living service which provides a step-down service for individuals moving on from Swan Court in addition to supporting individuals who are becoming more independent.
Falcon Mews has been designed to support 11 individuals with
Learning Disabilities, Autism Spectrum Condition, Mental Health needs and Behaviours that may be described as challenging to develop the independence, daily living skills and confidence necessary to move onto more independent living over time.
Sarah has mental health needs and moved into one of our supported living services. Sarah’s fantastic journey from hospital through to full independent living in her own flat in Hemel Hempstead.
Tailored Design
Key Design Features
- 11 spacious one-bedroom flats, alongside access to the communal living room and garden
- Each flat contains a bedroom and ensuite bathroom, living room/dining space and kitchen
There are a variety of flats to meet individual support needs and preferences.
Community integration
Falcon Mews is located between Moseley and Kings Heath, with excellent links to the local community and further afield. This plays an essential role in community integration, social skill development and facilitating positive relationships outside of the service.
L’Uccello Café – Social Enterprise
On site, we run the L’Uccello Café – a catering social enterprise service. In addition, we have developed a Skills Centre with horticulture poly-tunnels and a range of workshops to promote and increase skills.
Collectively, this provides opportunities for people to gain meaningful qualifications. L’Uccello Café Café and our linked Skills Centre are utilised by the people living across our new services, as well as other people who require support locally.
Our Multi-Disciplinary Team
Input from our in-house Multi-Disciplinary Team (MDT) can be provided on top of a person’s support when people we support require further therapeutic and behavioural support and intervention. This is an additional resource for people who have had input and cost agreed with the person supported and/or their funding authority.
Input can range from rehabilitative support for someone in crisis; support for complex assessments and transitions; resolving and reducing emotional and behavioural issues through positive behaviour support approaches; observing, training and mentoring support teams; right through to simply being on hand to give extra advice and guidance to our support teams.
Our specialised and highly qualified MDT includes Psychology and Speech and Language Therapy (SALT). We also work closely alongside external MDT partners including Behavioural Specialists, Psychiatry, Occupational Therapy, dieticians and fitness experts.
We work in partnership with local colleges in Birmingham to ensure that our services are responsive to the needs of young people and adults and are developed in line with educational and skills requirements – Debbie, Director of Operations
Referrals & Contacts
We provide a rapid response to referrals and will normally arrange a professional assessment within a few days of your call.
A full and detailed pre-admission assessment is always carried out, free of charge and without obligation, by our highly experienced team prior to a placement offering being made. This values the unique and complex needs of each person and forms the basis of the individuals support plans if they move into the service.
