Successful transition feedback highlights caring, supportive and empowering approach

We’re extremely pleased to share some feedback which was received from a social worker recently. The feedback came after one of our teams helped to transition someone from a secure hospital setting into supported living accommodation.
Due to circumstances beyond our control, the transition happened far quicker than originally planned. Despite this, the move has been a successful one, with the individual settling in and communicating well.
Skills and knowledge
“The staff at Precious Homes have demonstrated the skills and knowledge required to support this person who has complex needs.
While discharge planning was underway, an unplanned situation happened which resulted in the person needing to move as a matter of urgency. Precious Homes were able to facilitate this move urgently and managed to create a bespoke service in the person’s local area which they moved into during this summer.”
Caring, supportive and empowering
“Myself and others from our team have carried out visits to this new service regularly and have always been welcomed by the staff team. We have been able to carry out observations of staff support and interaction and we have been pleased with how they have supported the person from a restrictive environment to their own flat with a garden and plenty of community access.
The staff I have met have been caring, supportive, motivated and reflected on feedback from us in a positive way.”
Person-centred, PBS support
“We have witnessed person-centred support and seen how the staff have empowered the person to make their own choices in their own time. The person’s verbal communication has improved significantly since moving to their service. The person has not been subjected to any physical interventions or use of PRN medication (for behaviours of concern) since moving to the service and incidents are managed using a positive behaviour support model.
The team have listened to our requests and worked with the person’s family to support in meeting cultural and family needs too. The family are happy with how their child is being supported.
A professional approach
Precious Homes have in place all required risk assessments and support plans and have shared these with our team and supported us with making the application to Court of Protection for DOLS.
To add to this, in my opinion, Nicola and Ashleigh are a credit to the service, they are good at what they do. They have worked really hard to build good working relationships with us and I thank them for all their hard work.”
