Outreach Services

Our outreach support is offered on an individual basis and is undertaken by a small team of key workers who have been chosen because of their ability to meet the individual’s needs. They build strong relationships with the individual, supporting them to manage their own personal care needs, financial arrangements and everyday routines and activities.
