Our outreach support is offered on an individual basis and is undertaken by a small team of key workers who have been chosen because of their ability to meet the individual’s needs. They build strong personal relationships with the individual, supporting them to manage their own personal care needs, their financial arrangements, their everyday routines and activities and their integration into the community.

We help individuals to gradually develop confidence in their abilities, improve their interpersonal skills and change their behaviour.

We provide individual outreach services to people who live with their families, providing them with external activities during the daytime or to people who live in their own homes or tenancies and who require up to 24 hours support per day.

Our aim is to help individuals live an ordinary life in the community, regaining their self esteem and participating in activities which have often been absent from their lives for some considerable time such as going shopping, going to college or going on holiday.
